Аннотация: The aim of the study is to improve the results of uncomplicated
hypertensive crises treatment at the prehospital stage by
studying the state of systemic hemodynamics, determining the
balance of the autonomic nervous system, and markers relating
to the systemic inflammatory response in patients with hypertension
in uncomplicated hypertensive crises.
In 2017-2019, an open, prospective and comparative study
was conducted and consisted of 206 patients with documented
stage II HT, of which 131 patients applied for E.M.S. in the
“Municipal Institution Zaporizhzhya Station of Emergency
(Ambulance) Medical Assistance of “ of the Zaporizhzhia regional
Council and 75 people who had a stable course of the
disease and were examined on an outpatiently on the basis of the
“Primary health care Center No. 10”. Almost healthy 31 people
were examined on the basis of the MI “Regional medical exercises
dispensary” of the Zaporizhzhia regional Council.
The indicators of systemic inflammatory response among the
examined individuals in the study were analyzed. An increase in
the concentration of serum proinflammatory markers was demonstrated
depending on the stage and degree of BP increase.
Increased concentrations of catecholamines are associated
with increased blood levels of proinflammatory cytokines
such as IL-6 and TNF-б.the level of CRP may reflect
the inflammatory process in the intima of blood vessels.
Interleukin-1в (IL-1в), which is considered an “early response”
factor, has been proven to be involved in energy homeostasis

Аннотация: Вступ. Вибухові травми (ВТ) кінцівок залишаються однією з найбільш складних проблем сучасної військової
хірургії. Поєднання значного дефекту м’яких тканин із ушкодженням кісткових структур створює умови для розвитку
інфекційних ускладнень, нестабільності сегмента та формування хронічного остеомієліту. Сучасна тактика передбачає
етапне лікування з радикальною хірургічною санацією, тимчасовим заміщенням дефектів та подальшою реконструкцією
із застосуванням мікрохірургічних і клітинних технологій.
Мета – покращення хірургічного лікування вибухових
травм нижніх кінцівок за рахунок застосування клітинних
технологій. Introduction. To analyze the outcomes of staged reconstructive
treatment in a patient with a blast injury of the lower leg involving
combined soft tissue and tibial bone defects using a perforator
flap and cellular technologies.
Aim: to improve the surgical treatment of blast injuries of the lower
extremities through the use of cell technologies.
Materials and Methods. A clinical case of a serviceman with a blast
injury to the lower leg is presented. Radical surgical debridement
with resection of nonviable tibial segments was performed, followed
by placement of a cement spacer for temporary bone defect substitution.
Soft tissue coverage was achieved using a thigh perforator flap.
Tissue sampling was performed for osteoblast cultivation to create a

Аннотация: Представлена стаття присвячена актуальній науково-педагогічній проблемі – формуванню професійного мислення майбутніх лікарів, яке слід розпочинати з перших років навчання в медичному
університеті, коли студенти вивчають фундаментальні природничі науки. Продемонстровано, що
одним з інструментів вирішення цієї проблеми може бути застосування методу клінічного кейсу на
практичних заняттях навчальних дисциплін. Наведені приклади кейсів, які можуть бути використані при вивченні таких тем медичної та біологічної фізики, як «Ультразвук і його використання в
медицині», «Основи гемодинаміки» та «Біофізика зору»; наголошено на можливості створення
кейсів, які б відповідали усім темам курсу. Доведено, що застосування методу клінічного кейсу на
практичних заняттях має суттєві переваги у формуванні професійного мислення студентів, оскільки надає вміння аналізувати ситуації, що стосуються їх майбутньої лікарської діяльності, вчить
активно встановлювати причинно-наслідкові зв’язки, спираючись при цьому на уявлення, принципи
та закони фундаментальної науки. Таке навчання сприяє подоланню студентами психологічного
бар’єру перед фізикою, мотивує їхню пізнавальну активність, є запорукою глибокого розуміння фізичних та фізико-хімічних основ процесів життєдіяльності в організм людини, яке є необхідним у
подальшому вивчені фундаментальних і клінічних дисциплін. Застосування методу клінічного кейсу
при вивченні принципів роботи сучасного медичного обладнання на заняттях з медичної та біологічної фізики стимулює студентів докладати максимальних зусиль для вивчення відповідних тем,
що в майбутньому сприятиме вирішенню ними професійних задач діагностичного та терапевтичного спрямувань. Окреслено перспективи використання методу клінічного кейсу при вивченні фундаментальних дисциплін, що потребує визначення ролі кожної теми в професійній підготовці та
майбутній діяльності лікаря і створення відповідних кейсів. The presented article addresses one of the pressing scientific and pedagogical issues, the formation of
professional thinking in future physicians, which should begin during the early years of medical education,
when students study fundamental natural sciences. It is demonstrated that one of the effective tools for
solving this problem is the application of the clinical case method during practical classes.
Examples of clinical cases that can be used in teaching such topics of medical and biological physics as
“Ultrasound and Its Application in Medicine”, “Fundamentals of Hemodynamics”, and “Biophysics of Vision”
are presented. The possibility of developing clinical cases for virtually all topics of the course is also emphasized.
It is shown that incorporating the clinical case method in practical classes has significant advantages for
the development of students’ professional thinking, as it provides opportunities to analyze situations related
to future medical practice, teaches students to actively identify cause-and-effect relationships, and
encourages them to rely on the concepts, principles, and laws of fundamental science. Such an approach
helps students overcome psychological barriers toward physics, enhances their cognitive motivation, and
promotes a deep understanding of the physical and physicochemical foundations of vital processes in the
human body, which is essential for the further study of both fundamental and clinical disciplines.
The application of the clinical case method in studying the principles of operation of modern medical
equipment during medical and biological physics classes motivates students to make greater efforts in
mastering the relevant topics, which will subsequently facilitate their ability to solve professional diagnostic
and therapeutic tasks.
The article also outlines prospects for the broader implementation of the clinical case method in teaching
fundamental disciplines, which requires identifying the role of each topic in professional training and future
