Особливості показників ремоделювання лівого шлуночка у хворих на гіпертонічну хворобу

Abstract

Гіпертонічна хвороба займає на сьогоднішній день провідну позицію щодо захворюваності та розповсюдженості серед всіх серцево-судинних захворювань. Захворюваність на гіпертонічну хворобу у всьому світі носить характер пандемії. Мета дослідження – визначити особливості показників ремоделювання лівого шлуночка у хворих на гіпертонічну хворобу ІІ стадії. З метою реалізації дослідження було проведено комплексне обстеження 224 хворих на гіпертонічну хворобу, з них 126 мали II стадію та 98 I стадію захворювання. Верифікацію гіпертонічної хвороби проводили згідно наказу Міністерства охорони здоров’я України. Також були обстежені 35 практично здорові особи. Всі пацієнти були порівняні за віком і соціальним статусом. Ехокардіографію проводили усім пацієнтам на апараті «My Lab Seven» (Esaote, Італія) в М та В-режимах за стандартною методикою з частотою локації 1-5 МГц за загальноприйнятими методиками European Association of Cardiovascular Imaging, The American Society of Echocardiography. Hypertension currently occupies the leading position in morbidity and prevalence among all the cardiovascular diseases. The incidence of hypertension worldwide is a pandemic. The purpose of the study was to determine the characteristics of left ventricular remodeling in patients with hypertension of the second stage. Material and methods. In order to carry out the study, we carried out a comprehensive examination of 224 patients with hypertension, 126 of them had the second stage and 98 of them had the first stage of the disease. Verification of hypertension was carried out according to the order of the Ministry of Health of Ukraine. 35 almost healthy individuals were also examined. All the patients were comparable in age and social status. Echocardiography was performed in all the patients on the device "My Lab Seven" (Esaote, Italy) in m and b-modes according to the standard technique with the frequency of location equas to 1-5 MHz according to the generally accepted methods of the European Association of Cardiovascular Imaging, the American Society of Echocardiography. Results and discussion. The indicator of myocardial mass index between groups of patients with hypertension and stage and apparently healthy individuals had no significant difference (p > 0.05). While comparing the median with the value of this indicator in the group of healthy individuals equaled to 93.36 [88.43-105.44] g/m2 was highly significantly higher by 27.3 % in the group of patients with hypertension of the second stage (p < 0.05). The change in the relative wall thickness of the left ventricle was similar. Determination of myocardial mass index and relative wall thickness of the left ventricle made it possible to distinguish the types of left ventricular geometry in patients with hypertension of the second stage. It was determined that in the group of patients with hypertension of the second stage, eccentric and concentric left ventricular hypertrophy prevailed 49.2 % and 25.4 %, respectively.
