Автоматизація та гармонізація лабораторного аналізу сечі: порівняльна оцінка методів визначення глюкозурії

Abstract

Європейська настанова з аналізу сечі, яка розроблена Європейською федерацією клінічної хімії та лабораторної медицини є важливим кроком до вирішення проблеми відсутності гармонізації, що є актуальним не лише для України, а й для країн Європи. Згідно настанови для всіх аналізів сечі, що проводяться за допомогою тест-смужок використовуються рутинні методи, які у більшості випадків можуть забезпечити точніші результати скринінгу та остаточної діагностики. Метою дослідження є оцінка рівню гармонізації та діагностичної ефективності експрес-методу у порівнянні з рутинними методами, що застосовуються у медичних лабораторіях при визначенні вмісту глюкози у сечі. Матеріали та методи. Зразки сечі 105 пацієнтів були досліджені на вміст глюкози трьома способами – експрес-методом, глюкозооксидазним та гексакіназним методами. Статистичний аналіз включав кореляцію Спірмена, критерій Фрідмана та парний лінійний регресійний аналіз. The European Urinalysis Guideline, developed by the European Federation of Clinical Chemistry and Laboratory Medicine (EFLM), represents an important step toward addressing the lack of harmonization, which remains a relevant issue not only in Ukraine but also across European countries. According to the guideline, routine test strip methods are recommended for urinalysis, as they can, in most cases, provide more accurate results for screening and final diagnosis. Objective. The aim of this study was to evaluate the level of harmonization and the diagnostic performance of the rapid test method compared with routine laboratory methods used for the determination of urinary glucose concentration. Materials and Methods. Urine samples from 105 patients were analyzed for glucose content using three methods: the rapid test strip method, the glucose oxidase method, and the hexokinase method. Statistical analysis included Spearman’s rank correlation, the Friedman test, and paired linear regression analysis. Results. A significant positive correlation was established among the methods used for glycosuria determination. The highest level of agreement was observed between the rapid test strip method and the hexokinase method, confirming the diagnostic sensitivity of urine test strips. Comprehensive comparative analysis demonstrated that the hexokinase method produced results that differed significantly from both the glucose oxidase method (Z = 8.40; p < 0.001) and the rapid test strip method (Z = 5.51; p < 0.001). A correlation coefficient of 0.8544 indicated a strong linear relationship between the latter two methods. Conclusions. In clinical practice, only one analytical method should be used for longitudinal monitoring of glycosuria. Comparison or interchangeability of results obtained using different analytical methods is inappropriate due to the statistically significant differences between the laboratory methods demonstrated in this study.
