Досвід викладання невідкладних станів у дітей для лікарів суміжних спеціальностей на післядипломному етапі

Abstract

Курс «Невідкладні стани та інтенсивна терапія в педіатрії» призначений перш за все для лікарів швидкої допомоги як лінійних та педіатричних, так і спеціалізованих реанімаційних бригад, а також для лікарів-педіатрів, сімейних лікарів районних лікарень, які при виникненні критичного стану у дитини також повинні надати невідкладну допомогу та проводити адекватну інтенсивну терапію до приїзду спеціалізованої бригади. Проведення курсів тематичного удосконалення з питань невідкладних станів у дітей на робочих місцях дозволяє проводити навчання лікарів суміжних спеціальностей наданню невідкладної допомоги при критичних станах у дітей без відриву від основної роботи, що дає можливість більшій кількості практичних лікарів одномоментно пройти теоретичний курс та оволодіти практичними навичками, необхідними у їх повсякденній роботі. Курс «Неотложные состояния у детей раннего возраста» предназначен в первую очередь для врачей скорой помощи как линейных и педиатрических, так и специализированных реанимационных бригад, а также для врачей-педиатров, семейных врачей районных больниц, которые при возникновении критического состояния у ребенка также должны оказать неотложную помощь и провести адекватную интенсивную терапию до приезда специализированной бригады. Проведение курсов тематического усовершенствования по вопросам неотложных состояний у детей на рабочих местах позволяет проводить обучение врачей смежных специальностей оказанию неотложной помощи при критических состояниях у детей без отрыва от основной работы, что дает возможность большому количеству практических врачей одномоментно пройти теоретический курс и овладеть практическими навыками, необходимыми в их повседневной работе. The urgent medical care for children at the pre-hospital stage is often has crucial for the further forecast of their health status. This assistance usually turns by ambulance doctor, and quite often is not by specialized pediatric and linear team. At the same time the focus on the pre-hospital emergency medical care, a clear graded and algorithmization, resource allocation should significantly improve the willingness of doctors to work as well as the quality of emergency care for children in critical condition. To improve the theoretical and practical level of training doctors of related specialties at postgraduate stage for emergency assistance to children. In the course of Pediatric Anesthesiology and Intensive Therapy of the Department of Children’s Diseases of the Faculty of Postgraduate Education of Zaporozhye State Medical University, there was designed a cycle of thematic advanced training courses “Emergencies and intensive therapy in Pediatrics”. The course “Emergencies in Pediatrics” is intended primarily for ambulance doctors as linear and pediatric and specialized resuscitation teams, as well as for pediatricians, family physicians and anesthetists of district hospitals, which must provide urgent assistance for child in critical illness and to conduct adequate intensive care prior to arrival of the specialized team. Topical advanced training courses which are held in working-time with total duration – four academic hours twice a week for a total of 20 lessons. The time of trainings is agreed with the heads of medical hospitals, and planned in such way that treatment of child population mustn’t suffer. There are such forms of educational process on the thematic trainings as lectures, seminars and practical exercises. Lecture with multimedia presentation lasts two academic hours, seminars include discussion of medical topics, materials as well as the decision of test tasks. Practical classes are conducted by demonstrating clinical cases from the practice with using of multimedia presentation, photos of the child and the photocopies of results of examinations or other researches, training aid using models. Since the beginning of the 2012 at the course of Pediatric Anesthesiology and Intensive Therapy trere were introduced methods of teaching of practical skills like trainings ALS (advanced course of “resuscitation”), which are carried out by the European Council of Resuscitation. At these trainings, trainees learn to work in teams under the guidance of the leader, with maximum approximation to real clinical conditions, acquire the ability to quickly make a diagnosis and to carry out the necessary complex of measures in urgent situations. This principle we have implemented during practice skills of resuscitation with models. The cycle of thematic advanced training courses “Emergencies and Intensive Care in Pediatrics” consists of 5 main thematic blocks: “Emergencies in infants”, “Emergencies in a Pediatrc Allergology”, “Emergencies in cardio-vascular diseases in children”, “Emergencies of exogenous origin”, “Emergencies in Pediatric Gastroenterology and Surgery”. Thematical advanced training courses of pediatric emergencies in the workplace allows training of doctors of related specialties in emergency assistance during critical illness in children without departing from the basic work, that gives the opportunity to a big number of clinicians simultaneously undergo theoretical course and to master practical skills in their daily work.
